Don't ask me I can't remember how I did this.  Don't laugh, I was trying to do the end of a trumpet at the time.  I ended up with a pile of different shapes.  When I was popping them all I realised I had this nice toadstool shape.  Now I just have to remember how I did it.

Looks like you have the bubble going up into the top followed by a pinch twist, a small 2 inch bubble, pinch twist, With a loop that goes around the whole assembly and meets back at the second pinch twist. It then looks like you stuffed that part into a round and put a little bit of air in it. Well that is how I would duplicate the result at least.
